My doc made everyone sign one. Plus, every time anyone goes in for PM, they have to fill out a 2 page form about your pain level the last week, max, min, average, etc. The doc never looks at it. Personally, I think it's to cover their butts. There wasn't even room to write the name of the pharmacy I will use all the time on the form. I'm not sure if they can prosecute you, should they find out you get controlled from other places. I asked her what to do if I have to go to ER and get meds, and she said just tell her next visit. She didn't even mark my chart, just nodded. One time, I couldn't see her when I needed my Stadol, so I got a bottle at the urgent care center. I told her (prolly didn't have to) about it and said I needed to lower my monthly amount by one bottle. I wanted to be honest with her and I think she appreciated it.
