Sorry if this has already been said, I haven't scrolled all the way down this thread yet. But you can go to www.mymccard.com and they will tell you where to purchase them. The only place I've bought them is RiteAde pharmacy, but I'm sue there are others. Also, these can be reloaded (for a total of something like $500.00/time) and only cost like $5.00 each time. Of course there is also about a monthly fee of about $4.95.

Anyway, check out the website and get all the logistics, it's pretty easy and I've been doing it for the safety reasons as of late - meaning if there's no extra money on the card, they can't double bill you.
