Toe is quite right. The point of extended relief is to arrive at pain control and stay there so that you don't have to play "catch up". Once you are in pain it takes more medication to relieve the pain. This applies to chronic pain, where you can count on the constant pain recurring.
Even with acute painful conditions, it is better to take medication before your pain is out of control. If a person waits too long, it is very hard to acheive a comfortable level with normal doses.
So in the long run, you take less medication because you require less medication if you take it on a regular schedule. (Provided you have an adequate dose.)
